As a family physician who focuses on women’s health, I found this review to be a useful reminder that “stress” isn’t just a trigger word that we log and move past. It has measurable, physiological consequences for the clotting system that matter in everyday primary care, and it’s a mechanism I don’t think gets enough airtime in routine visits.

Stress, whether psychological or physical, profoundly affects the hemostatic system responsible for balancing blood flow and clot formation. Stress hormones like cortisol, adrenaline, and noradrenaline interacting with coagulation factors and platelets to promote a hypercoagulable state. Activation of the sympathetic nervous system releases other hormones, called catecholamines. These directly influence platelet function and vascular tone, while the HPA axis modulates clotting factor production through glucocorticoids like cortisol. In other words, both the “fight-or-flight” branch and the slower hormonal stress-response branch of the body join together to make blood clots more readily, a pairing worth explaining plainly to patients who assume stress only affects mood.
Psychological vs. Physical Stress
What’s clinically important here is that there are two stress types we see constantly in Family Medicine—psychological and physical stress. Psychological stress from anxiety, depression, and acute mental strain increases coagulation markers such as fibrinogen and D-dimer. This can raise thrombotic risk. Physical stress from intense exercise or trauma can also increase platelet reactivity and procoagulant factors. For a family doc, this reframes the anxious or depressed patient, not just as someone needing counseling or an SSRI, but as someone whose vascular risk may be quietly rising. This is also particularly relevant when layered onto other risk factors like smoking, obesity, or hormonal contraceptive use.
Chronic Stress and Cardiovascular Risk
Chronic psychological stress leads to sustained activation of the HPA axis and sympathetic nervous system. This results in continuous elevation of stress hormones and hypercoagulability. Clinically, this ties into the significant health implications of stress-induced hemostatic changes for cardiovascular disease — chronic stress contributes to persistent hypercoagulability, raising the risk of myocardial infarction (heart attacks) and stroke, while acute stress episodes can cause acute coronary syndromes, especially in people with histories of hypertension or diabetes. This is a good evidence-based talking point for patients who dismiss stress management as “soft” or “vague” advice — the mechanism linking chronic life stress to heart attack risk is biologically concrete, not just correlational, and framing it this way tends to land better than a generic recommendation to “relax more.”
When Stress Severely Disrupts Clotting
Severe physical stress such as trauma or burns can overwhelm the system entirely. This can lead to disseminated intravascular coagulation (DIC). DIC causes widespread clotting followed by bleeding. Although DIC is rare in routine primary care, it’s an important reminder for those of us covering OB-GYN, other areas of Women’s Health, or trauma-adjacent settings (postpartum hemorrhage, severe preeclampsia, major abruption) that the stress-coagulation relationship isn’t only about slowly increasing cardiovascular risk — it can worsen rapidly and unexpectedly, sometimes within hours rather than the days-to-weeks timeline typical of chronic risk.
Why Stress and Blood Clotting Matter for Women
Pregnancy, Postpartum, and Hormonal Contraception
Why does this matter specifically for women’s health? Because its mechanisms link directly to issues I manage daily. Pregnancy is already a naturally hypercoagulable state; layering the fibrinogen and D-dimer elevations described here onto a pregnant patient under high psychosocial stress, anxiety, or a traumatic delivery experience is a factor for venous thromboembolism risk, a concern that deserves attention along with the more established body of pregnancy-specific coagulation research already guiding our practice. Similarly, women managing combined hormonal birth control, migraine with aura, postpartum mood disorders, or perimenopausal anxiety are populations where high self-reported stress compounds other clotting risk factors, making it a real variable worth recognizing rather than ignoring it as background noise in a visit, particularly during counseling conversations about a patient’s choice of birth control.
What This Means for Clinical Practice
The take-aways for better practice include: (1) chronic stress and mood symptoms deserve to be framed to patients as cardiovascular risk factors, not just quality-of-life issues; (2) stress reduction (sleep, exercise, therapy, and pharmacologic treatment of anxiety/depression when indicated) has a large amount of support for being considered a part of cardiovascular and thrombotic risk reduction, not just mental health care; and (3) in higher-risk women — pregnant, postpartum, or on estrogen-containing contraception — significant psychological or physical stress is one more variable worth factoring into a patient’s individual risk conversations, especially when other prothrombotic factors are already present. None of this replaces standard risk assessment tools, but it does give us one more thread to pull on when a patient’s history doesn’t fully explain their presentation.
